To expedite check in, and reduce your wait time we encourage you to complete your intake prior to your visit. We understand that reading all consents can be time consuming and we want you to have the opportunity to read every word! This is why we happily provide the EZcheck in and Sadio portal.

To access the portal for the first time, you must use the link in the email we sent to create your username and password.
First verify your email: You should have received an email from noreply@ezderm.com to sign up for our patient portal.

How do I know if I need a referral to see a specialist?
If your member ID card states "Referrals Required," you'll need an electronic referral from your primary care provider (PCP) before seeking services from another network provider. A referral is when a PCP authorizes a covered person to see a specialist for diagnosis or treatment of a medical condition. Most often, that means you have to contact your PCP before seeing a specialist.
Not all health plans require a referral, but if your plan does, ask your PCP or clinic for an electronic referral before you visit a specialist. Without this referral, you may pay more or your care may not be covered.
If you don’t know if you need a referral, please call your insurance carrier directly.

We keep credit card information on file to ensure that patient balances are settled efficiently and with minimal hassle for you. This approach helps us reduce the number of phone calls, emails, and billing statements you receive, and it also prevents situations where minor balances result in collections notices. Rest assured, you'll always be informed of any balances due, and you'll have an opportunity to settle the payment before your card is charged. If you do not wish to keep a credit card on file we are happy to accept a $200 deposit that will be refunded to you after insurance has made payment on your visit.

No Show Fee of $75 will be charged to the credit card on file if an appointment is not cancelled at least 48 hours prior to the scheduled time. Arriving more than 20 minutes after your scheduled appointment time will be considered a "No Show." Certain appointments require a large deposit, such as allergy patch testing. If you do not come for these scheduled appointments, the deposit will become non-refundable.

An initial consultation for a new patient is $250. For new patients who need additional services, or for existing patients, the cost of an appointment with Dr. de Moll varies depending on the services provided. We can provide you with a detailed breakdown of our pricing. We accept major credit cards in person or via the patient portal. We cannot accept cash or personal checks at this time.”

It’s a great question—and the answer has to do with how insurance companies define preventive care versus specialist care.
Preventive Care Is used in Primary Care
Insurance carriers (including Medicare) only classify certain services as preventive.
These include annual wellness visits, physicals, and immunizations—services typically performed by primary care providers (family medicine or internal medicine doctors). Because dermatologists are specialists, our visits are not considered preventive, even when you’re coming in for something routine like a yearly skin check. Instead, dermatology appointments are billed as medical evaluations.

When you see us, we use Evaluation & Management (E/M) codes—such as 99203 or 99213—rather than preventive codes.
These codes reflect:
Why you came in
What we examined or treated
Any discussion, diagnosis, or follow-up plans
Even if your visit feels like “just a check,” it still involves a detailed medical evaluation, so it must be billed as such.
Why We Can’t “Switch the Code”
Sometimes insurance representatives suggest asking your doctor to “just change the code to preventive.” Unfortunately, that’s not possible for specialists.
Yes, codes are editable in a billing system when errors occur—but if we change a dermatology visit to a preventive care code, it will still be rejected because it doesn’t fit the guidelines for specialists. That leaves you responsible for the entire bill. So while the rep isn’t technically wrong that codes CAN be changed, they are often unaware of the consequences of doing so incorrectly.
If we submit a preventive code for a dermatology visit, your insurance will almost certainly deny the claim—meaning they won’t cover anything, and you could be responsible for the full bill.
We’re legally required to bill accurately and follow strict coding guidelines. Using the wrong code, even with good intentions, could create a bigger financial burden for you.
What About Skin Checks?
A skin exam might feel preventive, but it’s considered a medical visit because we’re actively looking for and addressing conditions such as skin cancer, rashes, or other concerns. That’s why it’s billed differently than your annual physical with a primary care doctor.
